What is SNS (Sistema Nacional de Salud) in Spain?

A project to settle in Spain, whether to live there all year round or part of it, involves much more than the acquisition of a property.
Expatriation also means integrating a new administrative, social and health framework. Among the steps not to be neglected, the question of health coverage deserves special attention, as it varies according to your status on site, the length of your stay and your specific needs. In Spain, the Sistema Nacional de Salud (SNS) provides a framework for access to public care for residents, and this is how it works.
The INSS : the essential step to access public health in Spain
The Instituto Nacional de la Seguridad Social (INSS) is the body that formalises affiliation to the Spanish social security system, which allows you to obtain a local social security number, which is essential to access the public health system, in addition to the NIE (Número de Identidad de Extranjero), which is required for all administrative procedures.
In concrete terms, the INSS does not provide care, but plays the role of administrative interface. It is this system that determines eligibility for public coverage, depending on your situation (professional activity, retirement, etc.). This right then opens up access to medical services, which are managed by the health services of the Autonomous Communities. Without this registration, access to public care (except for emergencies) may be restricted or not reimbursed.
The INSS also defines the basket of care covered by the SNS, the conditions of reimbursement (co-payment, out-of-pocket expenses), and issues the Tarjeta Sanitaria Individual, the personal health card of the insured.

The SNS : Spanish health system
Spain has a public health system, the Sistema Nacional de Salud (SNS), which is renowned for its quality and efficiency. It is based on a universal model based on residence, with decentralized management entrusted to the autonomous communities.
Access to this coverage is largely free of charge, but requires a contribution via social security contributions (employees, self-employed, etc.). This "universal but contributory" system allows access to primary, specialised and public hospital care, provided that you are affiliated. Once you have registered with the local health centre in your place of residence, you must choose a referring general practitioner, a necessary condition for benefiting from subsidised care.
Please note : unlike the French system, the distinction between the public and private sectors is very marked. Some structures offer both types of care: if the treatment is carried out in the private sector, no reimbursement is possible without private insurance.

Temporary stay or expatriation: what health coverage in Spain?
Short stay, permanent residency: health coverage options in Spain
For temporary stays (less than 90 days)
Travellers and temporary residents can benefit from public healthcare in Spain through the European Health Insurance Card (EHIC).
Valid throughout the EU and free of charge, it guarantees coverage of medically necessary care under the same conditions as Spanish insured persons. However, the EHIC is not suitable for long stays : it does not cover private care, specific care (dental, optical), or medical repatriation.
For more extensive protection, it is recommended to take out travel insurance, in addition to or as an alternative to the EHIC.
Private health insurance as an essential supplement
In addition, waiting times in the public sector can be long depending on the region. This is why some expats in Spain opt for local or international private health insurance. Expatriate contracts offer many advantages:
- Complete private care with the right level of reimbursement ;
- Continuity of coverage in the event of a temporary return to France or frequent travel;
- Greater freedom of choice for healthcare providers, particularly in terms of specialists or hospitals (without an imposed healthcare network).
- Guarantees of comfort such as osteopathy, and even teleconsultation (in one's mother tongue)

Moving to Spain means joining a health system that is different from the French model, where access to care is conditional on residency status and affiliation with the INSS. While French (or European) nationals benefit from a simplified procedure, the terms of care, particularly in the private sector, may be surprising.
Assessing your situation and needs allows you to adopt the best coverage strategy. Using international health insurance can be justified in Spain, where taking out private insurance is still essential for care in the private sector.
